What you’ll need:
- Resident/owner name and unit/address information
- Vehicle make, model, year, and color
- License plate number and state
- Any additional vehicle owners or household members’ vehicles
If you have multiple vehicles, you may need to submit the form separately for each one (check the form for instructions).
